Welcome!
Our receptionist will welcome you upon arrival and provide you with the appropriate forms to complete. It is important for the doctor to obtain accurate information regarding past and current health and medications. Please arrive 10 minutes early to complete the necessary paperwork for your first appointment. The new patient forms are also available to download from our website.
The initial visit will include a thorough history and physical examination as well as the report of findings, consent and usually a treatment. The initial visit usually takes about 30 minutes.
What to Bring
You do not need a referral to see a chiropractor. However, if you do have a referral or note from a medical doctor or other health-care practitioner, please bring it to your first visit.
Please bring comfortable, loose fitting clothing. Gowns and shorts will be provided if necessary. Please inform us and bring the appropriate information if this complaint is related to ICBC or WorkSafe BC.
